Insurances we are In Network with:
Kinesphere Physical Therapy LLC and its therapists are in-network providers for the above plans.
You will need to provide Kinesphere Physical Therapy with your:
*Insurance individual ID numbers (or social security # of sponsor)
*Date of Birth
*Customer service number on the card
With this information, as a courtesy, Kinesphere Physical Therapy LLC will call your insurance to verify your benefits prior to your first visit
Self Pay and Out of Network Options
Please call if you do not see your insurance listed above. We offer reasonable physical therapy prices for our expert services. If you would like to self submit to your insurance company, we can provide you with an itemized receipt that will provide you with needed codes. Many other insurance companies will reimburse for out of network expenses and/or apply it to your appropriate deductible. Payment is due at the time of service.
